What should I bring to my first appointment?
Please bring your insurance card, photo ID, copayment, medical records, and any radiology imaging you have had done (such as x-rays, CT, MRI). If possible, please print and fill out our new patient packet which is available at the bottom of our website.
Can I get the new patient paperwork online?

Does your clinic complete FMLA, DMV forms? how long does your clinic process records requests?
Records requests are processed within 24-48 hours upon receipt of the request. there is a customary fee imposed per page on these request.
What are the office hours and are they the same for each location?
North Buffalo Location- 8:30am To 6:30pm (Mon-Thurs) 8:30am-12:30pm (Fri)
Flamingo Location- 8:30am To 6:30pm (Mon-Thurs) 8:30am-12:30pm (Fri)

What is the cancellation policy if I’m late or miss an appointment?
Is my insurance accepted?
Although insurance verifications are routinely done, it is ultimately the patient’s responsibility to check with their insurance to verify coverage. You may also view the list of accepted insurance carriers below.
Does your clinic accept walk-in appointments?
Our clinics accept walk-in appointments on a case by case basis. Qualification for the walk-in appointments will be determined by office staff who may investigate patient eligibilities, referrals and reason for visit to our providers.
